Assumption of Risk and Release
University of Hawai‘i at Hilo Conference Center Assumption of Risk and Release related to field trips and activities associated with the “Evaluating the George W. Bush Presidency Conference, July 29 – August 1, 2009.
In the case whereupon a participant is below 18 years of age, a parent or legal guardian’s consent is both required and assumed by agreement of the following waiver.
I, the undersigned, in full recognition and appreciation of the dangers and hazards inherent in the field trips associated with the University of Hawai‘i at Hilo Conference Center and, during transportation to and from the stated trips, which I may participate in between July 29 – August 1, 2009, do hereby agree to assume all the risks and responsibilities surrounding my participation.
Further, I do, for myself, my heirs, executors, and administrators, hereby defend, hold harmless, indemnify, and release, and forever discharge the University of Hawai‘i at Hilo, UH Hilo Conference Center, and the Research Corporation of the University of Hawai‘i, and all their officers, and agents and employees from, and against, any and all claims, demands and actions or causes of action, on account of damage to personal property, or personal injury or death which may result from my participation, and which result from causes beyond the control of, and without the fault or negligence of the University, its officer, agents or employees, during the period of my participation aforesaid.
